Woxom Solutions LLC ("we," "us," or "our") operates the insurance premium quoting tool at quotes.woxomhealth.com (the "Service"). This Privacy Policy explains how we collect, use, store, and protect your personal information when you use our Service.
Our Service is used by licensed insurance agents to generate premium quotes for their clients, and by clients to view shared quotes, request quotes through agent portals, and submit enrollment applications.
Information We Collect
Quoting and Portal Information
When an agent generates a quote or a client uses an agent's portal, we collect:
- Full name, email address, and phone number
- Date of birth, gender, and tobacco use status
- ZIP code and coverage type
- Household income (optional, for ACA subsidy calculation)
- Spouse information: date of birth, gender, tobacco use (if applicable)
- Children information: date of birth, gender, tobacco use (if applicable)
Enrollment Information
When submitting an enrollment application, we additionally collect:
- Social Security Number (SSN) — only the last 4 digits are stored
- Full mailing address (street, city, state, ZIP)
- Payment information:
- ACH: Bank name, account type, routing number, account number (only last 4 digits stored)
- Credit card: Cardholder name, card number (only last 4 digits stored), expiration date, CVV (never stored)
- Electronic signature (typed name)
Agent Profile Information
Licensed insurance agents who register with the Service provide:
- National Producer Number (NPN)
- Name, email address, phone number
- Professional title and tagline
- License states
- Logo URL
How We Store and Protect Your Data

- Generate insurance premium quotes based on the information you provide

- Send email notifications to agents about quote requests and enrollments
- Display agent branding on public-facing pages (quote share pages and agent portals)
Who We Share Your Data With
We limit data sharing to what is necessary to operate the Service:
- Your insurance agent — The agent who created your quote, or whose portal you used, receives your quote and enrollment information
- Amazon Web Services (AWS) — Our infrastructure provider for data storage (DynamoDB) and email delivery (SES)
We do not:
- Sell your personal information to third parties
- Share your data with advertisers
- Share your data directly with insurance carriers through this tool (enrollment submissions are processed by your agent)
Data Retention

Children's Privacy
Our Service is not directed to individuals under the age of 18. We do not knowingly collect personal information from children. Dependent children's information (date of birth, gender, tobacco use) is collected only as part of a parent or guardian's insurance quote request.
